Neural foramen remodeling caused by a sequestered disk fragment.

چکیده
A 53-year-old man had had pain for 2 months in the left lower extremity radiating to the thigh . The pain had begun suddenly after he had lifted a heavy object. Clinical examination showed mildly decreased strength in the left leg, normal reflexes, and decreased sensation to pinprick in the left L3 nerve root distribution. A herniated disk at the L3 level was suspected, and myelography and CT of the lumbar spine were performed. The myelogram showed an extradural defect compressing the left lateral and posterior aspects of the thecal sac (Fig . 1 A). The postmyelography CT showed a soft-tissue mass in the left L3-L4 neural foramen. The mass mildly expanded the foramen and eroded the posterior margin of the vertebral body (Fig. 1 B). A neurofibroma was thought to be responsible for the findings, and the patient had surgical exploration. This revealed a tear in the posterior longitudinal ligament, and the mass was extracted in toto. Pathologic examination showed that the mass was composed of benign cartilage surrounded by granulation tissue compatible with a free intervertebral disk fragment.

Lumbar neural foramen remodeling by disk herniations.
The discussion of the case report by Dr. Castillo ( 1) of a neural foramen remodeling caused by a sequestered disk fragment prompts me to reply briefly . Dr. Castillo has written that bone remodeling or the neural foramen has not been previously reported in far lateral disk herniations. In an English-speaking paper (2) dealing with extraforaminal lumbar disk herniation (EFLDH), my coauthors and...

Background and Importance: Disk sequestration can be defined as a herniated disk with perforation of the fibrous ring (or outermost annulus fibrosus) and posterior longitudinal ligament with migration of the disk fragment to the epidural space. Even after the introduction of high-resolution magnetic resonance imaging (MRI), it is still frequently misinterpreted as neoplastic masses. U...
متن کاملCT myelography of a fragment of a lumbar disk sequestered posterior to the thecal sac.
The radiologic manifestations of lumbar disk herniation as seen on myelography, postmyelography CT, and, more recently , surface-coil MR have been well described (1-3). CT and MR findings of far lateral , extraforaminal disk herniation also have been reported (4]. Migratory fragments typically settle anterior and/or lateral to the thecal sac (5]. We report a case in which the sequestered migrat...

Forty-eight patients had 50 extraforaminal disk herniations (EFDHs) demonstrated on CT and/or MR by (1) presence of disk density or disk signal material lateral to the neural foramen, (2) displacement or obliteration of paravertebral fat, and (3) nerve root or ganglion compression or displacement. Forty-one of 50 EFDHs had a coexisting intraforaminal component; nine of 50 had an isolated far la...
